How do I request a replacement testamur?
Steps to apply, fee and document requirements, and processing information.
If you require a replacement testamur due to it being lost, stolen, damaged, or unavailable, please complete the Testamur Replacement Request Form.
Before submitting your request, please note:
- A $100 replacement fee applies. Payment is embedded within the form and will be processed at the end of your submission.
- A Statutory Declaration is required to support your request. Applications submitted without this document cannot be processed.
- Please allow up to five (5) business days for your request to be processed once all required documentation has been received.
